To create customer success, Morbark requires that all customers receive an instructional start-up explaining, safety, operation, and proper maintenance upon receipt of their new machine. The (SWV) form is specific to that family of machines and is to be filled out by both the technician performing the start-up and the customer/operator being instructed. It is also a check sheet that guides the technician through the start-up process. (NOTE TO DEALERS: You can download blank forms from the dealer portal.)
This is a multi-purpose form used to instruct the customer/operator on proper safety, operation and maintenance of their machine. Morbark uses this form to ensure the customer received good instructions on all aspects of their new machine. It is also used as a notification to Morbark to start the warranty on the customer’s new machine and engine, and triggers Morbark to send a follow up survey about the start-up to the customer.
A properly dated Start-up/Warranty Validation Form (SWV) must be completed, signed by the purchaser and returned to Morbark within seven (7) days of the start-up in-service of the equipment. Should the customer decline the instructional start-up there is a section at the bottom of the form that needs to be completed with the customer’s signature. This section documents the fact that the customer declined the start-up. This does not void their warranty; however it might help you and Morbark resolve a future dispute.

COMPLETE A SWV BEFORE EACH DEMO?
The real question is, why wouldn’t you?! Performing a Start-Up and Warranty Validation walk-around ensures the customer demo’ing a piece of equipment fully understands how to use it. That includes startup preparation, safety and maintenance of the machine. This form does not need to be submitted to Morbark’s Warranty Department until the sale is made but having a copy signed by the customer and dealership rep could help to protect the dealer rep, dealership and Morbark in the event there is an issue with the equipment while in the custody of the customer.
Another great reason to walk through this form with a customer is that it’s an opportunity to give them excellent customer service. If we really want to stand out from the crowd then it’s important we take advantage of every chance to demonstrate the highest level of customer care and service.

Step-by-step for Submitting Start-Up & Warranty Validation Forms
- You’ve just sold a piece of equipment. Way to go! While you’re working up all the paperwork to have your customer sign, this would be a great time to fill out the top portion of the SWV form so it’s ready to go when you deliver.
- When you deliver the equipment or your customer comes to pick it up, use the SWV form as you walk around the machine. It’s a great guide that will inform your customer the safe operating procedures for each feature.
- A scanned copy of the completed SWV form should be uploaded to the eCommerce Portal. Connect with your regional sales manager for instructions, if needed.
